Community Medicine Inc. (CMI) is a Federally Qualified Health Center Look Alike (FQHC-LAL). As an FQHC-LAL, we are able to provide services to qualified individuals and their families by offering:

Sliding Fee Discount:

A sliding-fee discount is designed to make our care more affordable to patients whose income is under 200% of Federal Poverty Level. Please enquire with our front office for Sliding Fee Discount Program’s application and instructions.

Does the discount apply to other services?
The sliding fee discount only applies to the services provided by CMI or by some of the organizations who we are contracted with CMI.

Who can apply for our Sliding Fee Discount?

• Any patient who visits our clinic is eligible for Sliding Fee Discount, if their income is less than 200% of Federal Poverty Level.
• Patients with high deductibles and co-payments can also qualify for Sliding Fee Program.

Are you without health insurance?
If you do not have any insurance, we can assist you by applying our sliding fee scale for office charges or/and help you apply Affordable Care Act (Marketplace/Obama Care) insurance or any program which you may be eligible for.
